The above pdf. files will allow you to view
the document on your computer and type your information right onto the
form before printing it out and mail. We suggest printing out two
copies of the same document so you have a copy for your records. Adobe
Reader is free - download it here.
Please be patient as your computer power and
internet connection will determine the download time.
Directions:
1. Select and click the appropriate
document.
2. If you don't have Adobe Reader on your computer, you can download it
free here.
3. To fill in blanks place your cursor on the appropriate area and left
click. The hand icon will turn into a vertical line so you can
type on to it.
4. For yes or no boxes that require checks hover
your cursor over the box and left click to place a check mark in it.
5. When completed click the "Print" Icon on the upper left of the
document.
6. Mail in the form.
